Online physiotherapy for sciatica

Sciatica is pain that travels from the lower back or buttock down the leg, usually because a nerve root in the lower back is irritated. Most sciatica settles with time and the right kind of movement. Physiotherapy finds the positions and exercises that calm the nerve, then rebuilds strength and confidence. That work is done through movement, which suits video sessions well. Sessions available in English, Telugu, and Hindi, and you can ask for a female or male physiotherapist.

Why online physiotherapy works for sciatica

The key early question in sciatica is which movements ease the leg pain and which make it worse. Your physiotherapist tests this live with you and builds the plan around what they find.

Sitting is a common trigger. On video your physiotherapist sees the chair, work setup and sleeping position you actually use, and can change what keeps irritating the nerve.

Leg pain often changes from day to day. Daily sessions let your physiotherapist adjust exercises quickly when symptoms flare or settle.

Getting in and out of a car or auto to reach a clinic is often the most painful part of the day for someone with sciatica. Online sessions remove it.

What we assess via video

Where the pain travels, and whether it goes below the knee
Movements that pull the pain back towards the spine or spread it further down the leg
Nerve tension tests you do with guidance, such as the slump test and straight leg raise
Strength checks: walking on heels and toes, single-leg sit to stand
Numbness and changes in sensation
Sitting, sleeping and driving positions

What treatment includes

Positions and movement strategies to ease leg pain in the first days
Directional exercises when a clear pattern is found
Nerve gliding exercises
A graded walking plan
Core and hip strengthening as pain settles
Advice on sitting, sleep, work and travel

How an online session works

1

Set up your space

Position your phone or laptop so your physiotherapist can see you move. You need about 2 metres of space.

2

Live assessment and treatment

Your physiotherapist watches your movement, corrects your form, and guides you through exercises in real time. Each session is 45 minutes to 1 hour.

3

Progress and adjust

Your physiotherapist tracks your improvement each session and adjusts exercises as you get stronger. You also receive exercises to practice between sessions.

What you need at home

A phone or laptop placed low enough to see you lying down and standing
A firm surface to lie on: a yoga mat, carpet or a firm bed
A chair without wheels
A towel or belt for some nerve exercises

What a typical course looks like

Sessions 1 to 3

Screening for warning signs, finding which positions ease the leg pain, and a short routine to do between sessions.

Sessions 4 to 9

Nerve glides, graded walking and gentle strengthening, adjusted as the leg pain eases.

Sessions 10 to 14

Strength, bending and lifting practice, and a plan to reduce the chance of it coming back.

This is a typical outline, not a fixed schedule. Your physiotherapist adjusts it to your progress and your doctor's advice.

Not sure where you stand? Take our free back pain self-check. It takes about three minutes.

When in-person care is better

Go to an emergency department immediately for numbness around the genitals or buttocks, new difficulty passing or controlling urine or stool, or weakness that is getting worse in one or both legs. These can be signs of cauda equina syndrome. See a doctor first if sciatica follows a fall or accident, or comes with fever, unexplained weight loss or a history of cancer. Every first session includes this screening.

Frequently asked questions

Can sciatica be treated without seeing a physiotherapist in person?
For most people, yes. Assessing sciatica relies mainly on your history, how the pain responds to movement, and strength and sensation checks, all of which can be done on video with guidance. Treatment is exercise and advice. If your physiotherapist finds signs that need a hands-on examination, a scan or a doctor, they will tell you in the first session, and you pay for that one session only.
How long does sciatica take to get better?
Many people improve noticeably within 6 to 12 weeks, though some take longer and recovery is rarely a straight line. Leg pain usually eases before full strength returns. Your physiotherapist tracks your pain, how far down the leg it travels and what you can do, so you can see whether you are on track.
Should I rest in bed with sciatica?
Short rests in a comfortable position can help during a bad flare, but long periods of bed rest tend to slow recovery. The aim is to stay as active as the pain allows and use positions that ease it. Your physiotherapist will work out what that looks like for you.
Do I need an MRI before starting?
Usually not. An MRI is mainly needed if surgery or an injection is being considered, if there are warning signs, or if symptoms are not improving after several weeks of good treatment. Many people with no pain at all have disc bulges on MRI, so a scan alone does not decide treatment. If you already have one, share the report on WhatsApp before your first session.
