Online physiotherapy after ACL reconstruction

ACL rehabilitation is long, usually nine months or more before returning to sports with jumping and sudden changes of direction, and it works in stages. Each stage has clear goals to meet before moving on. Most of the work is exercise you can do at home or in a gym, guided and checked by your physiotherapist on video. Sessions available in English, Telugu, and Hindi, and you can ask for a female or male physiotherapist.

Why online physiotherapy works for ACL reconstruction rehabilitation

Early ACL rehab is daily knee movement and thigh muscle work, which is easy to guide on video and hard to keep up if you have to travel

Later stages use squats, lunges, hops and landing drills. Your physiotherapist watches your knee position on camera and corrects it

Sessions can happen at home, in your gym or on a ground, wherever the current stage needs

Rehab continues for months. Online sessions fit around college or work and make a long programme realistic

What we assess via video

Knee straightening and bending
Swelling and thigh muscle activation
Walking, and whether you still need crutches or a brace
Squat and lunge control: does the knee drift inward?
Single-leg balance and strength
Hopping and landing control in later stages

What treatment includes

Early knee movement and swelling control
Thigh and hamstring strengthening
Balance and control training
Running progression once your knee meets the criteria
Jumping, landing and change of direction drills
Guidance on return-to-sport testing

How an online session works

1

Set up your space

Position your phone or laptop so your physiotherapist can see you move. You need about 2 metres of space.

2

Live assessment and treatment

Your physiotherapist watches your movement, corrects your form, and guides you through exercises in real time. Each session is 45 minutes to 1 hour.

3

Progress and adjust

Your physiotherapist tracks your improvement each session and adjusts exercises as you get stronger. You also receive exercises to practice between sessions.

What you need at home

A phone or laptop propped up to show your whole body
About 2 metres of floor space
A resistance band
A step or stair
Later on, access to a gym or open ground if possible

What a typical course looks like

Weeks 0 to 2

Full straightening, bending, swelling control and waking up the thigh muscle.

Weeks 2 to 12

Normal walking, strength and balance.

Months 3 to 6

Running and harder strength work, once your knee meets the criteria.

Month 6 onwards

Jumping, cutting and sport-specific drills, then a gradual return to sport, usually not before nine months.

This is a typical outline, not a fixed schedule. Your physiotherapist adjusts it to your progress and your doctor's advice.

When in-person care is better

Contact your surgeon urgently for fever, a wound that is red, hot or leaking, a swollen painful calf or sudden breathlessness. If the knee gives way with a pop and swells, see your surgeon. Formal strength testing before returning to sport sometimes needs equipment found in a clinic or gym, and your physiotherapist will tell you what is needed.

Frequently asked questions

Can ACL rehab really be done online?
Most of it can. ACL rehab is progressive exercise with clear goals at each stage, and your physiotherapist can watch your movement, count your repetitions and correct your form on video. If a stage needs testing equipment, your physiotherapist will tell you what is needed and where to get it.
When can I return to cricket or football after ACL surgery?
Usually not before nine months for sports with jumping, cutting or sudden stops, and only after you pass strength, hop and movement tests. Returning earlier is linked to a higher risk of injuring the knee again. Your physiotherapist works towards those criteria rather than a date on the calendar.
Are 21 sessions enough for ACL rehab?
No. 21 sessions cover the early weeks, when close supervision matters most. Rehab continues for months after that, and you can continue with additional sessions at the same per-session rate. Your physiotherapist will plan the later stages with you.
