Does health insurance cover physiotherapy in India?
Health insurance coverage for physiotherapy in India is limited and inconsistent. Here is what the major insurance types cover, what they do not, and how to access affordable physiotherapy regardless of insurance status.
Private health insurance
Most private health insurance policies in India (Star Health, HDFC ERGO, ICICI Lombard, etc.) cover physiotherapy only when it is part of an inpatient hospitalization. If you are admitted to a hospital for surgery or a medical condition and receive physiotherapy during your stay, it is typically covered under your policy.
Outpatient physiotherapy, the kind most people need (visiting a clinic or having a therapist come home for back pain, knee pain, or recovery after discharge), is generally not covered by standard health insurance plans. Some newer comprehensive plans and top-up policies are beginning to include outpatient benefits, but coverage is limited and often capped at low amounts.
If you have a policy that claims to cover physiotherapy, read the fine print carefully. Many policies require a doctor's prescription, limit the number of sessions, or cap reimbursement at amounts well below actual costs.
Government health schemes
CGHS (Central Government Health Scheme) covers physiotherapy for central government employees and pensioners at empanelled facilities. Coverage includes outpatient physiotherapy with a referral from a CGHS-registered doctor. However, finding empanelled physiotherapy clinics can be challenging, and wait times may be long.
ECHS (Ex-Servicemen Contributory Health Scheme) provides physiotherapy coverage for retired armed forces personnel at empanelled hospitals and clinics.
Ayushman Bharat (PM-JAY) covers physiotherapy only as part of inpatient treatment packages at empanelled hospitals. Standalone outpatient physiotherapy is not covered.
State government insurance schemes vary. ESI (Employee State Insurance) covers physiotherapy at ESI hospitals and dispensaries.
Corporate and group health plans
Some employer-provided group health insurance plans offer better physiotherapy coverage than individual policies. Large IT companies and multinationals sometimes include outpatient benefits or wellness allowances that can be used for physiotherapy.
Flexible benefit plans or wellness wallets offered by some employers may allow you to allocate funds for physiotherapy. Check with your HR department about what your specific plan covers.
Even when covered, corporate plans often have session limits (typically 10 to 20 sessions per year) and may require pre-authorization.
The reality: most physiotherapy is out-of-pocket
For the majority of Indians, physiotherapy is an out-of-pocket expense. This makes it important to find quality treatment at a reasonable cost rather than delaying care while navigating insurance bureaucracy.
Online physiotherapy makes this more accessible. Without clinic overheads and travel costs, online sessions are often more affordable. And because a good physiotherapist teaches you exercises to do independently, the total number of paid sessions needed is typically lower than you might expect.
Many patients find that 6 to 10 focused sessions, combined with a home exercise program, resolve their condition. At typical online physiotherapy rates, this represents a manageable investment for significant pain relief and functional improvement.
